1. Application;

2. Specification (more than 1 item, 1 with a price, 2 without a price);

3. Acts on the warehouse and quality;

4. A copy of the Charter of the enterprise;

5. Copies of constituent documents (EGRPO, etc.);

6. Copies of certificates for measuring instruments;

7. Copy of the safety certificate;

8. A copy of the lease agreement or government act;

9. Copy of technological regulations;

10. Copy of cost estimate for products;

11. A copy of the approved product composition (for concrete products);

12. List of equipment (name, brand, year, country, quantity);

13. List of normative documentation;

14. List of employees (full name, position, education, work experience);

15. Copies of the contract for raw materials and contracts for quality;

List of documents required for certification of services:

1. Application;

2. Constituent documents of the Applicant:

For a legal entity - duly certified copies of constituent documents, an extract from the unified state register of legal entities of Turkmenistan, a certificate;

For an individual - a duly certified copy of the document on state registration as a private entrepreneur;

3. Information about the Applicant's employees (surname and name, year of birth, position, work experience, education, information about other knowledge acquired related to the specialty);

4. Information about the equipment, means, devices used to perform the relevant service;

5. If measuring instruments are used, a copy of the document confirming the passage of state verification in the prescribed manner;

6. List of normative legal acts and normative documents regulating the services performed;

7. Information about the monetary turnover of services, certified by the signature of the head of the Applicant;

8. Copy of the safety certificate;

9. A copy of the lease agreement or government act;

10. A copy of the certificate from the Sanitary and Epidemiological Station;

11. Information about tariffs for services (Price list)
